Nurse Navigators to Support Neonatal Transition: Fabiana Bacchini Shares Insights with CHILD-BRIGHT

In a recent CHILD-BRIGHT Network webinar, Fabiana Bacchini, Executive Director of CPBF, joined a discussion on the challenges families face when transitioning from the NICU to home. The episode, “Nurse Navigators to Support Neonatal Transition,” highlights the urgent need for better psychosocial support for parents of infants with medical complexities.

Parents in the NICU often experience elevated stress, anxiety, and depression, and these challenges intensify during the move from hospital to home. While clinical care is usually well-coordinated, support for parents’ emotional and psychological needs is still limited.


The webinar introduced Coached, Coordinated, Enhanced Neonatal Transition (CCENT) a study evaluating a nurse navigator (NN)-led intervention designed to better support families during this period. Through CCENT, nurse navigators help guide parents through the transition, addressing both medical coordination and psychosocial needs.

Participants learned to:


  • Understand the emotional and psychological challenges parents face during the NICU-to-home transition

  • See how integrated care models like CCENT improve mental health outcomes for families and high-risk infants

  • Explore how incorporating a navigator role can strengthen parental support during this vulnerable time


Fabiana’s participation emphasized the importance of improving family-centered support and ensuring parents are not left to navigate this journey alone.
